The Complication Everyone Is Frightened Of
Almost nobody gets it. Almost everybody having wisdom teeth out spends a week terrified of it, counting days and inspecting the hole with a phone torch.
Read enough posts from people recovering from wisdom teeth and one thing dominates every single thread. Not the surgery. Not the swelling. Not the money.
Dry socket.

That last line is the whole phenomenon in one sentence. The fear is doing more work than the pain.
What it actually is
When a tooth comes out it leaves a hole in the bone. Blood fills it and clots, and that clot is the entire dressing — it covers exposed bone, protects the nerve endings, and forms the base the new tissue grows over.
Dry socket is what happens when that clot is lost, dissolves, or never forms properly. Somebody on r/wisdomteeth described it more precisely than most leaflets:

Exactly that. The pain isn’t infection and isn’t damage. It’s raw bone and unshielded nerve endings sitting in an open mouth, meeting air, water, food and temperature. It typically shows up two to four days after the extraction — the recovery gets better, and then abruptly gets much worse.
And it is genuinely severe. This is not a minor complaint being oversold:

But the fear is far more common than the thing
Here is the part that reframes it: most people never get it. It’s an uncommon complication, and it’s most likely after lower wisdom teeth, particularly impacted ones.
Yet fear of it structures the entire week for almost everyone:

Both of those people are describing ordinary recovery from lower extractions and both have jumped straight to the complication. That happens constantly, and it’s understandable — nobody hands you a way to tell the difference.
So here’s the difference
Ordinary recovery hurts most in the first two days and then improves, unevenly, with bad hours inside better days. The pain sits in the jaw and the muscles as much as the socket. Painkillers help.
Dry socket goes the other way. Days one and two are fine or improving, then somewhere around day three or four the pain climbs sharply and doesn’t respond to the tablets that were working. It often radiates toward the ear on that side. Cold air and cold water hit it specifically. There’s frequently a bad taste or smell that arrives with it.
The single most useful marker: which direction is it going? Recovery trends better. Dry socket turns and gets worse.

Broadly right, and it’s the most reassuring fact in the whole subject. The risk lives in the first few days, while the clot is doing its job alone. Once tissue has begun growing across, there is no clot left to lose.
So the week of vigilance has an end date, and it’s sooner than the anxiety suggests.
What genuinely reduces the risk
Don’t smoke or vape. This is the strongest single factor, and people in the threads already know it — that’s what the person choosing fear over their vape was doing. Suction and heat both work against the clot.
Nothing through a straw. Same reason. Negative pressure in the mouth is exactly what lifts a clot out.
Don’t rinse hard for the first day. Gentle salt-water rinses are usually advised from the second day onward. Vigorous swishing on day one is the opposite of helpful.
Eat soft and cool, and don’t poke it. Every inspection with a fingernail or a phone torch is a chance to disturb something that’s holding fine.

There is a recognized association with oral contraceptives, and the difference between men and women is a real observation rather than a rumor. Not a reason to change anything — just a reason not to assume you did something wrong.
If it does happen
It is fixable, quickly, and that is the part the fear leaves out. A dentist cleans the socket and packs it with a medicated dressing. Relief usually arrives within minutes to hours, and the dressing gets changed a couple of times while it heals.
It’s a short appointment, not a second operation. The one thing not to do is wait it out at home for a week on painkillers that aren’t touching it.
