What Still Works at 3am
Saliva stops overnight and doesn't come back until morning. We tried five products aimed at that window and judged them on the same two questions.
Every dry-mouth thread eventually arrives at the same question:

Most answers treat it as a comfort problem. A hygienist in another thread pointed out why that’s the wrong frame:

Saliva isn’t just moisture. It rinses acid away and carries the minerals that repair enamel all day. Take it away for eight hours and the repair half of the cycle stops while nothing stops the damage half.
The two questions we judged everything on
Comfort and protection are different jobs, and almost every product on this shelf is sold for the first one. So we tried five of them against the same two questions:
1. Is it still there at 3am? Anything you rinse, swallow or spit is gone in minutes.
2. Does it do anything for the enamel while it’s there? Wet is not the same as protected.
Ranked worst to best.
5. A glass of water by the bed
Does nothing on either count. Water is gone in a minute, carries no minerals, and doesn’t hold moisture in the mouth. It also means waking up to drink it, which is its own problem.
Fine as a reflex. Not a plan.
4. Biotène and the other moisturizing gels
The default recommendation, and the one that generates the most disappointment in patient communities:

I can’t tolerate Biotene-like products at all. I feel like they’re more suited for those in the earlier stages of the disease. r/Sjogrens
Worth noting that people who need these most have moved on to alternatives:
Biotene changed their formula. The best gel out there is Salivea and I swear by it! My dental hygienist recommended it r/Sjogrens
Question 1: partly — a gel outlasts a rinse, not a night. Question 2: no. These are lubricants. They make a dry mouth more bearable and leave the enamel exactly where they found it.
3. XyliMelts and the adhering discs
A genuine step up on staying power. The disc sticks to the gum and dissolves slowly, so it’s still working hours later, and the xylitol in it is something the cavity bacteria can’t ferment into acid.

Not for everyone, and people are candid about the trade:
the drugs do not last long enough for comfort and the xylimelts have borked my taste buds. A small price for comfort. r/Sjogrens
Question 1: yes, clearly. Question 2: no. It holds moisture and it starves the bacteria, which is real, but nothing in it goes back into the enamel.
2. Prescription-strength fluoride gel
The one thing on this list a dentist will actually write up for a dry mouth, and it’s on the list because it answers the second question properly. Brushed on last thing and not rinsed off, it stays on the tooth and it drives mineral back into the surface.
Question 1: yes. Question 2: yes.
Two catches, both practical rather than scientific. It needs a prescription, so it isn’t something anyone starts tonight. And it’s a brush-at-the-sink routine — which is exactly the routine that collapses on the nights this problem is worst.
1. CariSpray
Answers both questions, and neither catch applies.
Question 1: a spray that isn’t rinsed off, so it goes on last and is still there in the morning. No sink and no routine to collapse, which matters on the nights nobody made it to the bathroom.
Question 2: nano-hydroxyapatite is the mineral enamel is built from, small enough to settle into the surface rather than sit on it. Xylitol alongside it does what the discs do — the bacteria can’t ferment it.
Top of the list because it’s the only one that answers both without a prescription.

If the goal is getting through the night comfortably, most of this shelf works. If the goal is having the same teeth in ten years, the list gets very short.
Questions people actually ask
What is the best thing for dry mouth at night?
Depends what the goal is. For comfort, the gels and lozenges are fine. For the teeth, only two of the five do anything at all — the ones that put mineral back on the enamel while the mouth is dry.
Does Biotène work for dry mouth?
For comfort, for some people, briefly. The recurring complaint in patient communities is that it doesn't last long enough to cover a night, and it isn't aimed at the teeth.
